The ASEA/AFSCME Local 52 Health Benefits Trust understands that medical information about you and your health is personal, and we are committed to protecting your privacy by protecting that information. The Health Insurance Portability and Accountability Act of 1996 (HIPAA) was signed into law to improve the efficiency and effectiveness of the health care system in the United States. HIPAA includes standards that help ensure the privacy and confidentiality of patient information for you and your family.

HIPAA regulations restrict the types of individual employee health information available to the ASEA Health Trust’s staff, and may require that you contact the customer service representatives of the health plans directly if you have questions or require specific health information.

The customer service contact information for each plan is available in the Key Provider Contacts section of this site. To protect an individual’s private health information, the health plan’s customer service department may require you or a covered family member to sign a HIPAA Release Form before health information can be released to your spouse or chosen representative.
