The ASEA Health Trust’s Chronic Kidney Disease Management program is provided through Renalogic. It is voluntary, completely confidential and free. This program is available to all participants of the ASEA Health Trust (employees, spouses and dependents).
1 in 3 adults is at risk of developing chronic kidney disease. Chronic kidney disease (CKD) is a “silent disease”, because most people aren’t aware that they have the disease before it’s too late and they have to start dialysis. Finding out that you have CKD or end-stage renal disease (ESRD), which can result in dialysis, is a life-changing event. But, we’re here to help.
Fortunately, kidney disease is reversible, and preventable, if identified early enough. Renalogic is the ASEA Health Trust's Chronic Kidney Disease Management program provider. Renalogic works with the Trust to educate its members, and their enrolled family members, about chronic kidney disease (CKD) & end-stage renal disease (ESRD). Renalogic’s Nurse Coaches specialize in kidney health, but also teach members how to improve their overall health including diabetes and hypertension management, two of the most common conditions that lead to chronic kidney disease.
How to Use the Program
This program is a part of your benefits with ASEA/AFSCME Local 52 and is provided at no cost to you. Renalogic Nurse Coaches will reach out to members who have been diagnosed or are at high risk for CKD or ESRD. Renalogic periodically reviews the ASEA Health Trust's claims data to identify people who may be at risk of having chronic kidney disease. As a part of this health plan, your participation in this program is voluntary and completely confidential.
