The Trust’s Dental Plan benefits are provided through and administered by Delta Dental of Alaska (DDAK). You can receive services from any dentist, but your out-of-pocket expenses may be lower if you choose a dentist who belongs to the Delta Dental PPO Network or Delta Dental Premier Network. Remember, the annual Plan maximum resets every year on July 1st.
  • DDAK provides you with your Dental ID card. You should provide your ID card to your dental provider for any dental treatment. If you have not received your Dental Plan ID card, please contact DDAK at 888-374-8906 (toll free), Monday-Friday, 6:30 a.m.-5:00 p.m.

The ASEA Health Trust’s Dental Plan covers diagnostic and preventive services, basic and major restorative services, as well as prosthetics such as inlays, crowns, implants, bridges and dentures. The Dental Plan covers orthodontic treatment for enrolled adults and children at 50% up to a $1,500 lifetime maximum per enrolled person.

If your dental provider is not able to submit a claim for their service; use the Request for Reimbursement – Medical/Dental Benefits to submit a claim.

Amount Covered by Plan Option

The amount the Plan pays for dental care is based on the Allowable Expenses of the Medical Plan Option in which you are enrolled.
