ASEA Health Trust Benefits
- If your dependent no longer meets the eligibility rules, the Plan cannot cover him or her. Review the Eligibility section of the Plan Booklet to see if your dependent child is eligible for Health Trust benefits.
- If your dependent’s status changes, it may affect your Health Trust benefits. See the Changing Benefits section of this website and review the Qualifying Events section of the Plan Booklet.
- If you want to change your Health Trust benefits, submit a new Flexible Benefits Enrollment Form.
- If your child is covered under another group health plan (e.g. your spouse’s), the Health Trust will coordinate benefits with the other plan. To learn about this provision, see the Coordination of Benefits section of this website. Please complete the Other Coverage Statement Form to provide information about their coverage.
- Make benefit changes within the required number of days after the qualifying event. Submit a new Family Information Form within 60 days of the qualifying event to inform the Health Trust of marital and/or dependent status changes and your new benefit choices. If you wish to change your plan selection, submit a new Flexible Benefits Enrollment Form.
- If your dependent child ceases to be eligible or gains eligibility for other benefits, you have these options:
- If your dependent child ceases to be eligible for benefits, he or she may be eligible for COBRA Continuation Coverage. See the Continuation Coverage section of the Plan Booklet for eligibility details and enrollment instructions. You must notify the Health Trust within 60 days of the status change to continue self-pay coverage through COBRA.
- If your dependent child gains eligibility for health benefits through his/her own group plan, you may dis-enroll him/her from the Health Trust plan. Submit a Family Information Form to update your current list of dependents. If you wish to change your plan selection as a result of this change, submit a Flexible Benefits Enrollment Form.
